In a national poll about health issues (CDC Project), 12,638 U.S. adults were surveyed and it was found that 6413 of those surveyed had health issues. At a 5% level of significance, perform a hypothesis test to test the claim made by the television news story. What is the correct conclusion? I get a Z Score of 0 not sure where to proceed from there

For a one-tail test you should get $\displaystyle p=0.047$. The two-tail test is double that. Are you sure you have the right formula? $\displaystyle z=\dfrac{\widehat{p}-p}{\sqrt{\dfrac{pq}{n}}}$
