How do I write my non-linear model in a paper?

I have made a non-linear model in Excel I want to write it professionally and generically in my paper. How would you write it professionally?

I have attached what I have so far. It is just too specific maybe?

If you can write it like it would be written in a research paper, I will reward you with a litecoin.

Thank you
